The worst time to design a process is during the incident
When an AI system does something it should not have, three clocks start at once, being the harm clock, the notification clock and the record clock. Organisations that improvise lose all three, because the people managing the harm are also inventing the process and reconstructing the record afterwards from memory and chat logs.
